Big news! In addition to new group enrollment, CaliforniaChoice Census Enrollment is now available for in-force group enrollments, member changes, terminations, and waivers. Check out the new training video and updated Census Enrollment Guides with detailed instructions.

Census Enrollment Templates

Here are the templates you need for Census Enrollment for new and in-force groups.

• New Groups: 7/1 Census Enrollment New Business
• In-Force Groups: Census Enrollment Change Request

Employer-Level Changes

The Census Enrollment Change Request form cannot be used for employer-level changes. If your group would like to make employer-level changes, it must complete and submit the Employer Change Request Form.
