We offer a variety of Medical and Ancillary enrollment kits on our website to make it easier for you to process your new group enrollments.
While materials vary somewhat by carrier, health plan, and administrator, you can expect to submit some or all of the following in connection with your group’s enrollment:
- Group census
- Employer or Master Group application
- Employee applications
- ACA Banking Consent Form
- Wage and Tax Statement
- Primary Care Physician (PCP) selection form – for HMO plan enrollees
- Producer Agreement or licensing paperwork
- Case Submission
Submission of all required forms will help expedite approval. If you’re unsure of what’s required, contact your Word & Brown representative or the Account Management team.
