We’re excited to announce that CaliforniaChoice^® is launching Spreadsheet Enrollment, which allows new employer groups to submit our template in lieu of paper employee enrollment applications!
 
Who is it for?
Spreadsheet Enrollment is for CaliforniaChoice new employer groups. For those who prefer paper applications, they’re still available for use!
 
How does it work?

1. A Spreadsheet Enrollment Template is completed with employee and dependent enrollment and/or waiver information in lieu of paper applications.
   • 7/1-12/1/2025 Template
   • 1/1-3/1/2026 Template
2. Employer group must complete and submit the Group Attestation Form, if using spreadsheet enrollment for submission.
