Our goal is to process your new group enrollment easily and efficiently in order to provide you and your client with a quick approval. The following list outlines Delta Dental (MWG)'s case submission requirements.
Employer Application / List Bill Agreement
Authorization Agreement for Automatic Payments with copy of voided check made payable to “Morgan-White Administrators, Inc.”
Employee Enrollment Forms – minimum two employees enrolled. If employee wants vision, see page 4 of the Delta DFE Brochure for the two plans.
Licensing: Broker appointment must be completed. NOTE: The group will not be approved until licensing is received.
